Human Oversight Principles

For every AI system:

Human accountability remains mandatory.
AI outputs shall be reviewed before significant business use.
Human operators may override AI outputs at any time.
AI recommendations are advisory unless explicitly approved by a human.
Significant incidents shall be reported and investigated.

Review Requirements

This register shall be updated whenever:

a new AI system is introduced;
Human Operator changes;
oversight responsibilities change;
operational context changes;
regulatory requirements change;
significant incidents occur.
