AI Incident Reporting Declaration

Document Information

1. Purpose

This declaration confirms that Firmă demonstrativă has established, implemented and maintains a documented Incident Reporting process for Artificial Intelligence systems in accordance with Regulation (EU) 2024/1689 (EU AI Act).

The declaration demonstrates management commitment to ensuring that AI-related incidents are identified, reported, investigated, documented and resolved in a consistent and traceable manner.

2. Declaration

Firmă demonstrativă declares that:

• AI-related incidents are identified and reported without undue delay;
• every reported incident is documented and assigned a unique Incident ID;
• incident investigations include root cause analysis;
• corrective and preventive actions are implemented where necessary;
• objective evidence supporting each incident is maintained;
• incident records are available for internal audits and regulatory inspections.

3. Management Commitment

Management commits to:

• maintain an effective AI Incident Reporting process;
• allocate sufficient resources for incident investigation;
• ensure timely implementation of corrective actions;
• periodically review incident trends;
• continuously improve AI governance based on lessons learned;
• support regulatory inspections and external audits.

4. Scope

This declaration applies to:

• internally developed AI systems;
• externally acquired AI systems;
• proof-of-concept AI systems;
• production AI systems;
• AI infrastructure;
• all personnel using AI systems on behalf of Firmă demonstrativă.

5. Review Frequency

Incident Reporting arrangements shall be reviewed:

• annually;
• after every significant AI-related incident;
• after major AI system changes;
• after regulatory updates;
• before regulatory inspections.

The organization commits to maintaining complete, accurate and traceable incident records, investigating every significant AI-related incident, implementing corrective actions and continuously improving the AI governance framework.
